Case Management

Our case management program is designed to help older adults find the services they need to remain in their own home and community. In a typical case, the case manager first completes an assessment that evaluates the physical, financial, environmental, cognitive, and functional levels of the older adult in need.

The case manager and the individual then develop a plan for what services are needed based on the results of this assessment. The case manager arranges for the services and adjusts them according to the ongoing needs of the older adult.

Our case managers are trained and caring professionals who are knowledgeable about issues related to aging and about community resources available for older adults.
